How to Get or Use This Software

Limited Audience

This software is purchased via a Master Service Agreement negotiated by Procurement and various U-M units purchase different bundles of licenses via order forms against the MSA. This group includes Dearborn ITS and Ann Arbor ITS. The cost is a sample; cost varies by the number of licenses purchased.

May not collect credit card or bank account numbers.

Max liability 3x the annual payment. For DPA, liability is equal to fees paid during the term of the agreement.

Consortium/Funding Info

There is a master contract between the University and FormAssembly; various units including Dearborn and Ann Arbor ITS pay for their own licenses as part of this agreement. There is also a Data Protection Agreement in place.

Individuals at U-M are ultimately responsible for any infringing software on their computers or devices or for violating the terms and conditions of software licenses. Individuals are responsible for reviewing the SPG on Software Procurement and Licensing Compliance.
